Next check that you have firmware v1.07 installed on your controller:

I also suggest running through the following:
- Test another USB cable, best to try a *brand new* cable and test all USB ports.
- Try adjusting your USB buffer size to 10ms or higher
- If you're using USB hub, ensure this is plugged into it's AC adapter if it has one
